This form must be used by teachers who have witnessed any incident involving a student(s) for which suspension or expulsion MAY be considered (including swearing at a teacher, uttering threats, possessing drugs/alcohol, being under the influence, bullying, vandalizing, weapons, sexual assault, activities “motivated by bias, prejudice or hate”, etc).
All staff members are required to complete this form and submit it to your administrator.
These forms MUST be available to any staff from the main office at your school. Forms are numbered and are required to be completed within 24 hours of any incident that you are witness to.
Similarly, admin is required to respond to each and every form that is submitted – you will be informed in writing within 24 hours what is being done in response to the incident that you have reported.
